In Vitro Fertilization (IVF): When is it indicated?

In vitro fertilization (IVF) is indicated in the following cases:
  • Women with obstruction in the fallopian tubes.
  • Endometriosis.
  • Women with a limited egg reserve.
  • Sterility of unknown origin.
  • Ovulatory disorders.
  • Couples with severe male factor.
  • Couples who have not achieved pregnancy using other simpler techniques.
  • Single women or female couples.

What is In Vitro Fertilization?

In vitro fertilization (IVF) is a technique in which eggs are fertilized in a laboratory to obtain embryos. After cultivating the embryos for a few days, they are transferred to the uterus to achieve pregnancy.

The assisted reproduction technique to be used will depend on the examinations and tests carried out in the fertility study prior to any treatment: 

  • IVF
    In vitro fertilization is a laboratory technique that allows an egg to be fertilized with a sperm outside the uterus.

How is IVF treatment performed?

In vitro fertilization (IVF) is carried out in the following phases:

1. Ovarian stimulation
Ovarian stimulation aims to increase the growth and development of follicles in the ovary. Through the controlled administration of hormones, a greater number of eggs are obtained than those obtained in a normal menstrual cycle, which increases the chances of obtaining embryos. This phase is controlled by ultrasounds and hormonal analyses and lasts approximately 8 to 12 days.

2. Oocyte retrieval
Once the follicles have reached the optimal size, the egg retrieval is scheduled. This is carried out under sedation in about 15 minutes. It is a painless procedure that does not require hospitalisation, and after one or two hours of rest, the patient can return home. It is carried out with the help of an ultrasound machine and a special needle through the vagina. The retrieved eggs are stored in the laboratory awaiting insemination.

3. Sperm capacitation
In this phase, the semen sample is prepared in the laboratory to improve its fertilization capacity. A process is carried out in which the best quality sperm are selected and treated to ensure their capacity to fertilize the egg.

4. Fertilisation
In the embryology laboratory, the eggs are joined with the sperm by means of “classic” IVF or ICSI (Intracytoplasmic Sperm Injection). In classic IVF, the egg is placed together with the sperm in a drop of culture medium. In ICSI, a sperm is selected and introduced directly into the egg using a micropipette. The fertilised eggs are kept in incubators with optimal temperature and oxygen conditions.

5. Embryo culture
After a few days in the incubator, the embryos are assessed daily. This phase is key to selecting the best quality embryos for transfer, with the aim of achieving successful implantation in the uterus. Embryo quality is essential for the success of the treatment.

6. Embryo transfer
After the embryos have been cultured and selected, patients are informed about the number and quality of the embryos available. The transfer takes place in the embryology laboratory in just a few minutes and is a painless procedure that does not require anaesthesia. The selected embryos are placed in a fine cannula and deposited in the uterine fundus with the aid of an ultrasound scanner. Surplus embryos are vitrified for possible later use.

Approximately 14 days after the transfer, the pregnancy test is performed.
